I'm convinced it's an HDMI handshake issue and it only happens when I connect to a receiver and then to a screen - both via HDMI and also Thunderbolt to HDMI adapter. There were lots of posts in 2011 & 2012 on the subject (I've read a LOT of them) but they all seem to dry up by the end of 2012 so I get the feeling there was a fix for the issue but I can't seem to find it. I have also tried booting with Mountain Lion, Mavericks and Yosemite and none made a difference. I've also already tried multiple SMC as well as PRAM resets which haven't worked.
